ABSTRACT

The evaluation of the oral health-related quality of life (OHRQoL) is important for clinical assessment and could be an indicator of the quality of the treatment received. Objective: To evaluate the OHRQoL in adults with removable prostheses in relation with patient characteristics such as age, sex, type and time of use of the prosthesis, previous experience and perception about the stability of the prosthesis. Materials and Methods: 217 patients from the Dental Clinic of the National University of San Marcos-Peru were evaluated after rehabilitation with a removable prosthesis. A structured questionnaire was applied by telephone interview. The evaluation of the OHRQoL was carried out using the General/Geriatric Oral Health Evaluation Index (GOHAI). For the statistical analysis, the Mann Whitney U test, the Kruskal-Wallis H test and the Spearman correlation were used. Results:The sample included 63 men (29%) and 154 women (71%) with an average age of 66.34. The average GOHAI score was 52.44+8.15. The GOHAI score was related to age (p=0.241), sex (p=0.110), type of prosthesis (p=0.069), previous experience (p=0.293), and perception of movement of the prosthesis (p<0.001). Conclusions: The GOHAI score indicates a moderate quality of life related to oral health after prosthetic rehabilitation. The GOHAI index can be considered effective for the evaluation of the OHRQoL.


La evaluación de la calidad de vida relacionada a la salud oral (CVRSO) resulta importante para la valoración clínica pudiendo ser un indicador de la calidad en el tratamiento recibido. Objetivo: Evaluar la CVRSO en adultos con rehabilitación protésica removible relacionándola con características de los participantes como edad, sexo, tipo y tiempo de uso de la prótesis, experiencia previa y percepción sobre la estabilidad de la misma. Materiales y Métodos: Se evaluaron 217 pacientes de la Clínica Odontológica de la Universidad Nacional Mayor de San Marcos-Perú, posterior a la rehabilitación con prótesis removible. Se aplicó un cuestionario estructurado mediante entrevista vía telefónica. La evaluación de la CVRSO se realizó mediante el índice de Evaluación de Salud Oral General/Geriátrica (GOHAI). Para el análisis estadístico se utilizaron las pruebas de U de Mann Whitney, H de Kruskal-Wallis y correlación de Spearman. Resultado: La muestra incluyó 63 hombres (29%) y 154 mujeres (71%) con una edad media de 66.34. La puntuación media del GOHAI fue de 52.44 + 8.15. Se relacionó la puntuación del GOHAI con la edad (p=0.241), sexo (p=0.110), tipo de prótesis (p=0.069), experiencia previa (p=0.293) y percepción de movimiento de la prótesis (p<0.001). Conclusion: La puntuación del GOHAI indica una moderada calidad de vida relacionada a la salud oral después de la rehabilitación protésica. El índice GOHAI puede considerarse efectivo para la evaluación de la CVRSO.

ABSTRACT

Objectives: The level of knowledge, awareness, and attitude about teeth-replacement options among a group of medical and paramedical subjects and to compare them with the general population. Materials and Methods: A self-administered questionnaires using simple Arabic language were distributed to two groups of subjects. Questions focused on the willingness to replace the missing teeth, the preferable methods of choice for replacement, and the reasons for these choices. The first group (G-I) was from the medical and paramedical staff who work in a military hospital at Jordan Royal medical services, the dental staff was excluded from the study. The other group (G-II) was from the general population who attended the dental department in the same hospital with comparable level of education. All the participants were partially edentulous excluding the third molars. Clinical examination was done by qualified prosthodontist to evaluate the possible prosthetic treatment options for replacement. A total of 612 questionnaires were distributed, of which 533 questionnaires were returned (response rate 87.09%). The results were analyzed and comparison was made between the two groups. Results: Responses to questions about awareness and attitude about prosthetic management of missing teeth revealed that G-I have more awareness than G-II to the probable causes for tooth/teeth replacement and limitation of the preferable method for replacement (P<0.05). More than 80% of the participants believed that replacement of anterior teeth is more important than the posterior teeth. Implants and fixed partial denture (FPD), respectively, were more preferable than removable prosthesis, although clinically was not indicated in cases (P<0.05). There was no clinical benefit from replacement of missing teeth in 33.4% while only 6% believe this. Conclusions: This study showed that the awareness and attitude between the medical and paramedical staff to prosthetic needs is better than between general populations. The demands for dental replacement by patients were significantly different when compared with the actual needs.

ABSTRACT

Ectodermal dysplasia is a hereditary syndrome characterized by dysplasia of tissues of ectodermal origin (hair, skin, nails, and teeth) and occasionally, dysplasia of mesodermally derived tissues. The triad of nail dystrophy (onychodysplasia), alopecia, or hypotrichosis (scanty, fine, light hair on the scalp and eyebrows) and palmoplantar hypohidrosis is usually accompanied by lack of sweat glands and partial or complete absence of primary and permanent dentition. Hypohidrotic ectodermal dysplasia usually has an X-linked inheritance and affects only males severely, while female heterozygotes show only minor defects. The clinical management of children with ectodermal dysplasia provides a unique opportunity for cooperative effort between the pedodontist and the prosthodontist. The following case report discusses the management of a young boy with hypohidrotic ectodermal dysplasia. Removable prostheses were employed in the treatment. The aim was to rehabilitate the adolescent prosthodontically and boost him psychologically.

ABSTRACT

Objetivo: Avaliar, por meio do indicador subjetivo Oral Health Impact Profile, versão simplificada (OHIP-14), o impacto dos problemas bucais na qualidade de vida dos agentes comunitários de saúde (ACS) que trabalham na Rede Pública de Saúde do Município de Vitória-ES. Método: Trata-se de um estudo transversal que utilizou quatro roteiros: seleção dos participantes, perfil sociodemográfico e condição bucal, estrutura da prática odontológica e o OHIP-14 que mensura o impacto gerado por problemas bucais na qualidade de vida. Foram incluídos os agentes comunitários de saúde que trabalham no Programa de Agentes Comunitários (PACS) do município de Vitória, entre 19 e 60 anos. Os dados foram coletados na própria unidade de saúde de cada grupo de agentes, pela pesquisadora e por uma auxiliar de consultório dentário devidamente treinada, entre setembro e dezembro de 2006. A amostra final foi de 238 ACSs. A análise estatística utilizou os testes Qui-quadrado, odds ratio (OR), Mantel Haenszel e regressão logística. Resultados: A maior percepção de impacto aconteceu para as pessoas mais jovens, de ambos os sexos, com menos escolaridade e dentadas, mas principalmente para aquelas com necessidade declarada prótese parcial removível (PPR). Conclusão: O impacto produzido pelas condições bucais na qualidade de vida das pessoas pode ser determinado pelo sindicadores socioodontológicos e a associação entre medidas normativas e subjetivas possibilita uma análise mais lógica e precisa, podendo redirecionar todas as políticas de gestão dos sistemas de saúde, para que o mesmo se volte às reais necessidades do público que deve assistir.
